INTAKE AND OUTREACH REPRESENTATIVE The Center for Elders Independence is a non profit organization serving frail elders in the East Bay . Our innovative, all-inclusive, long-term health care program (PACE Program) provides the frail elderly with all the care they need, allowing them to maximize their health & independence and remain close to their family members and within the community. We currently have an excellent opportunity for a full-time Intake and outreach Representative. The Position : Under the direct supervision of the Intake Manager, the Intake and Outreach Representative responds to calls from people inquiring about CEI program services and assists prospective participants through the intake process. Additionally, the Intake and Outreach Representative attends outreach events such as health fairs and conducts presentations to increase the awareness of CEI within the community and generate referrals.
